Measuring for Custom Socks
I came across an interesting article on Measuring for Custom Socks by Apple Leef Farm. It give a nice diagram of where to measure and a work sheet.
It assumes that you know the basics of knitting socks/hose but the work sheet would be good to have for your records.

I have a notebook for socks, kilt hose, with measurements for everyone that I've knit for....so far, Tommy and I have had the most use of the notebook, then my granddaughter comes in a close second to me!
I take a lot of measurements so that I can knit a sock that precisely fits the person it is made for. My measurements for kilt hose are listed like this
Circumference below the knee
Circumfrence around the widest part of the calf
Circumfrence of the foot around the ball of the foot
Circumfrence of the ankle at the narrowest part
Distance between the calf and the narrowest part of the ankle less one inch
Distance below the knee to the widest part of the calf less one inch
Distance below the knee to the narrowest part of the ankle
Length of foot less 1/2 to 1 inch-depends upon how snug you want your sock to be
