Actually the material that kilt is made out of is designed for medical garments (much heavier than scrubs) and it has a sort of protective coating on the threads. It stays cleaner than you might think, and most of the stains that do end up on it will wash out without much trouble.
It's designed to be resistant to blood and bodily fluids, so its pretty tough stuff.
