Some Asian split reed instruments, such as the gya ling, which is used in Tibetan Buddhist rituals, are played with the reed outside the intrument and inside one's mouth. It seems to work better that way with the continuous breathing method of playing, where one inhales and exhales at the same time, using the cheeks like the bag in a bagpipe to force the air out through the reed.
