I flew on SWA a while back and wore my kilt. I've also flown back and forth to America on KLM, NWA, Continental and Delta and I had absolutely no issues with any flight ever. The staff were always polite and friendly. My kilt only ever brought compliments.

I've never heard about any of these incidents. I might have to have a look for them.

    (Edited to add.)

Honestly, I think the waitress shouldn't have got any trouble at all.

As for the two 18 year old girls... Hmm. I made a point of reading 13, (count 'em, thirteen!) different articles about the incident and with each one I felt more and more like being pretty had little or nothing to do with it.
If someone were giving me unfair treatment on a flight I certainly wouldn't assume it was because they were jealous about me being young or handsome.
