After twisting my ankle while riding my bike last year, using toe clips and having worn running shoes on, I removed the clips. Now just have plain platform pedals. I may eventually switch to clipless pedals, but, I know that I might forget to clip out when coming to a standstill, which would cause the result mentioned in your original post. At least, in my case, I didn't fall, but my foot bent in an unusual manner, causing the damage.
