Glitch
Let me assure you Barb Tewksbury is still making kilts, I own two of her creations, and could not recommend her highly enough. here are my reviews of both kilts.

I'll add my recommendation for Barb T. to the list, or, since you're already looking at the Scottish Tartans Museum for a jacket and waistcoat, how about considering them for your kilt as well? The 8 yard kilts are made elsewhere, but Matt makes the 4 yard box pleats himself. In fact, there's a Gunn box pleated to the red stripe on his frontpage. $395 + shipping versus $412 + international shipping (and possible customs fees).

Free shipping does not necessarily mean Royal Mail from GB. It might be UPS or Fed-Ex who are their own brokers and pass on exorbitant fees. With Royal Mail or USPS or Canada Post you might get tagged for duty but pay a minimal service charge. It's something one would have to demand at point of purchase if not previosly offered. With all that being said go with Barb. I have seen her work first hand (McMurdo's kilts) and have found them top quality and very reasonably priced (with the majority of that the cost of the material).
