When I was growing up, My father made sure he taught us not to wear hats inside. I wore a hat to dinner one time. Aftrer dad saw it, I have NEVER worn a hat inside. To this day, when I go shopping and am wearing a hat, I leave it in the truck when I go into the store. Old habits are hard to break, and I know Dad is still watching.

Ladies are not required to remove hats at all, it is entirely their discretion. Men are required to remove hats when indoors unless in uniform and performing duties related to the uniform, defined as being "under arms". For example, a military detail performing funeral honors does not remove their covers, even if the funeral is held indoors.
I'm not sure about any modes of civilian dress, but there are several items of U.S. Naval Full Dress that are not worn often and when they are, the wearer is considered to be under arms, whether performing official duties or not. Examples would be the officers' swords and enlisteds' cutlasses. These should be removed upon or before entering a house of worship, unless you are performing duties related to your branch of service. Example: If a friend wants Full Dress for his wedding and asks me to be a groomsman, I'd wear a cutlass (assuming the other groomsmen were, too). Showing up as a guest with a cutlass would be a no-no. I'd be required to remove it both by ettiquette and Naval regulation before entering the church.

"Required"? Required by whom? Men SHOULD remove their hats indoors, including when in uniform, but I'll let you military sorts debate that one. This "under arms" phrase is quite odd, actually.
Gentlemen certainly can wear swords in church. Of course officers, being gentlemen, do the same AT CERTAIN TIMES AND EVENTS. Bridegrooms can wear swords; judges, sherrifs, gentlemen at certain state religious ceremonies, coronations, etc, etc. wear them. If US Navy regs prohibit the wearing of cutlasses by warrant officers, then so be it, but please don't make blanket assertions that just aren't true.

I just follow the very simple rule of:
"If rain can hit my head, hat on. If not, hat off".
This rarely fails me in my military profession or when wearing civilian attire. I too have a mild dislike of folks wearing hats indoors (and sunglasses), particularly when they are large and obscuring my view of a movie or event. I accept that many folks wear a ballcap from the moment they rise to the moment they fall asleep no matter what and will probably never make an issue of it unless I'm teaching a class or something. Quite frankly it is so rare to see a woman wearing a hat these days I wouldn't even know what to say

There is a quote "Never tell your Grandmother how to suck eggs" because she knows everything. Today, many people wear what they want when they want, But with the kilt headdress is definately taken off.
