I used to work there.
People rarely showed up in costume, but it did happen.
Unless you pack in your own cutlery (and why would you, you'd only be made a mockery of) you eat with your hands.
The environment is filthy and loud. Don't wear anything you care about.

Whenever I hear "Medieval Times", I always think of this scene from "The Cable Guy":
Steven: Can I get a knife or fork?
Wench: There were no utensils in medieval times, hence there are no utensils AT Medieval Times. Would you like a refill on that Pepsi?
Steven: There were no utensils but there was Pepsi?
Wench: Dude, I got a lot of tables.

I'd think if one did kilt up, the staff would certainly have some fun with it.
