I didn't wear my first kilt until I was 30, and start wearing it to a local bar that I would stop at on my way home kept the kilt, and after a month on only wear it to the bar i ventrued out from there.

I live in a college town and I think that makes wearing a kilt out both easier and harder, easier because the town is used to seeing things that are crazy or out of the norm, harder cause of the density of population composed predominately of frat boys who think they are cool. you might have him try some heavy hiking or combat boots with a simple gray or red rag sock scrunched down and a basic black tee-shirt (I know you said he was trying to lose weight and black is slimming.) that basic combination with a really simple black sporran is about as masculine and confidence inspiring as you can get I think,
