I agree with the whole on ramp thing, but I'd like to add those who drop to 10 or often 20 miles below the speed limit when they see the sign that their exit is a mile or two ahead.
And how about those who pull out cut you off, often ignoring the stop sign and the fact that there is no one behind you, on a rural road with no passing zones and drive 20 or more Mph under the speed limit. I had that one happen to me yesterday. Gallager, the comedian, had a suggestion for that one. All vehicles would be equipped with a device that applied a stamp to the offending vehicle. When the cops saw a vehicle with three of the stamps. they would pull the driver over, take his license, and tow the car.

yep!
You must be clairvoyant or be legitimately annoyed by people who do this, as I am.

Idiots that hang in the left lane and drive the same speed as the right lane. The left lane is for passing, not sightseeing.

When I was commuting, I noticed a LOT of drivers would stay in the left lane, even when people were passing on the right! I think it's some sort of mental block where they think the left lane will always move more quickly so they want to be in it, even when that's obviously not the case.
What really gets me are the people on the freeway who are just creeping along, often below the minimum speed. When you look at them, you can tell they are absolutely terrified to be in the traffic. I always wonder why they out in it. If you have to travel at that time, there are almost always alternate routes that aren't moving so fast. But I guess that falls under another of my irritations, people who only know one route to get to their destination and have been travelling that way for years without learning the alternatives.

Here's a few more major annoyances.
My cat when it brings in field mice. He doesn't eat them, just plays with them. I have traps EVERYWHERE because they often get away from him.
The same cat, when he gets wet in the rain he comes inside and expects to be dried off immediately. The bad part about this is, if it's at night, he gets in bed with me. NOTHING wakes you up faster than a wet cat curled up against your back.

I hate:
- finding peanut-butter boogers in the jelly/jam jar.
- drivers who cruise in the left lane.
- drivers who do not speed up on the on ramp.
- drivers who pass me, get in front of me, then slow down.
- mushrooms - it is a fungus and they have no taste
- always getting seated in the worst places in restaurants cause you have kids with you regardless of their behavior.
- seeing SUV's capable of hauling 15 tons and climbing mountains, only being used to transport little Jake to soccer practice.
- Mazda commericals.
- Valentine's Day.
- Paris Hilton.
- The ridiculously high salaries of professional sports players.
- Lobbyists.
- Kemper Auto and Home Insurance.

Re: Bathrooms
The handicapped stall is roomier so that we can fit wheelchairs in there, not so some lard a** can stretch out while moving his/her bowels.
Also- that's also where they tend to put diaper changing stations... it ticks me off when I walk into a bathroom with my (stinky) toddler and someone (non-handicapped) takes that one in front of me when there's others available. Also- men's rooms in family friendly places that don't have changing space, or make you guys do it on the counter, cheeses our family off because we're equal-opportunity stinkbusters. But I digress...
I hate:
When I forget to eat anything until *looks at clock* 2:53 pm and know I'm going to ruin my own dinner
My backyard's drainage, which seems incapable of taking care of an inch of rainwater a week
When I ask a bartender for a hot toddy and they put the whiskey in the cup instead of to the side- then microwave it! Grr!
People who think all their opinions are facts. Including me when I do it.
I could probably think of more, but I gotta eat something. Now.

Don't get me started on traffic issues that I hate. I knew our city had crappy drivers, but I never realized just how crappy until I started driving a 40' bus around for Transit.

On the flip side, how about the "entitled" attitude of smokers, who think it's their "right" to spew burning trash into the air the rest of us are trying to breathe? Not all smokers are like that, but too many of them are.
Fortunately, New York has laws against smoking in restaurants, shops, etc.
Having a smoking section in a restaurant is like having a peeing section in a pool.
