For me, it depends on a couple of factors: I tend not to go regimental in public in my AK's as they're not quite as tight around the upper pelvis as my SWK's are. And, as has been mentioned elsewhere in this thread, it depends on the activity. If we're just out walking about, I don't feel it's an issue. But, if I'm going to be performing an activity that could leave me flashing someone, I'll typically put something (color matched briefs, usually) on underneath.
Last week out in Oregon, I was out on the beach with the wife and daughter, and had squatted down to take some pics at daughter-height, and my wife informed me that it was very obvious from the front that I was, in fact, regimental that day. Fortunately, we were pretty much on the water, and the only folks behind her were in Japan.
I started thinking a bit more about it at that point.

Here's my view of it,
I've been going going regimental and commando for more then 10 years. the only time I wear underwear is when I train and play rugby. Those shorts are short...
The first day is akward and uncomfortable, but by the 3rd day, you don't notice anymore...
Save on laundry... and get yourself another kilt...

Regarding modesty & going regimental: Utilikilts are great for preserving modesty when squatting, kneeling, etc. due to their small front apron and pleats that come all the way to the front of your legs - the apron has lots of extra fabric to allow it to drop between the legs.

I'm coming at this from the other side actually - I've only gone regimental twice, and it was at home only.
Now, I don't really have a problem with anyone going regimental - I don't care what anyone (doesn't) wear under their clothes (I won't be lookin'), and if an accidental exposure happens then it's forgivable (IF it's accidental). Personally I think it's really annoying how people can be such prudes about something as natural as a human body (even though it may not be a pretty sight sometimes, it's still natural and something we all should be acquainted with).
The reason I tend to not be regimental is because most of the times that I wear my kilts, it's to work. And when I come home, well, I still live with my parents and it's kinda close-quarters here. When I go out, I just worry about all the prudes.

Okay, another thing for me to get used to - you call it regimental, we call it "going commando" on this side of the pond.
As far as wearing pants (undies to you), I've never worn them with my kilt. The only time I embarrassed myself was at a wedding once when I did a lively twirl when dancing and the whole room got a view of the family jewels! I got a round of applause - thank heavens it was late in the proceedings and most folks had had a few bevvies by then...

For the first few days after getting my kilt, I played it safe, but after I tried going regimental there was nothing that could compare and I've been as such since, regardless of questions and prying eyes. I figure if people really want to know that much, and are brave enough to try, then what they see isn't my problem. I'm also careful when it comes to accidentally exposing myself, and have only had a few minor incidents so far.

If you wear a sporran, the weight of it should be enough to keep the front of the kilt in place. With the back, however, it's a different matter - but people don't seem to have as much of a problem with that!

I'm in much the same boat as many of you.
I started off feeling strange not to wear the undies, then my GF basically dared me to go commando...I did so and never went back. The feeling of freedom and comfort is hard to describe. I actually feel kinda like society has been keeping me down all these years, lol. About the only time I don't go regimental these days is if I am going somewhere with lots of kids or to Church.
I totally agree with the comment on the Utilikilts, (especially the workmans, the heavy canvas rules) they are awesome for modesty's sake, plus I love the way they 'jaunt' as you walk. If you are new to going undieless, a utilikilt is a great way to break that barrier.
