The largest crowd where I didn’t expect to see anyone else kilted was 1844 people at the Nashville Symphony.

Stone Mountain Highland Games or Grandfather Mountain, whichever has the largest attendance.

But then, there were those cruises. How many people on a cruise ship? I was, for certain, the only one kilted either time.

Would likely be this 4th of July when I led my audience of about 1000 down to River Street to join a crowd of about 20,000 to watch the fireworks.

I've been to the Stone Montain Highland Games (and other Games here & there) a couple of times. Lots of people, lots of kilts, so nothing really unusual in that regard.
I'd have to say, though, that the largest group of people looking at me in a kilt (with most folks in "regular" clothes) was when the Louisville Pipe Band opened for the opening act for K.C. & the Sunshine Band when they played at the Louisville Waterfront Park a couple of years back. Maybe 5000-6000+ people? I emceed the pipe band's performance and played on a couple of tunes.

Hartfield-Jackson Atlanta International Airport
Fourth of July Weekend at the Georgia Aquarium
Fergus Scottish Festival
Dona Ana Renaissance Craftfaire, Las Cruces, New Mexico
Those would be the top four, I think.

Every Highland Games I compete in; The Opera, Symphony, Detroit Tigers Baseball games, Renn Fests, Chicago Inst. of Art...hmmm, seems I'll wear a kilt anywhere!

Whilst the numbers may not quite be as great as those of the Gathering, I expect that the total number of kilted attendees at the 2005 world massed pipe band record breaker Pipefest in Edinburgh in which I took proud part as a member of the 1st Bohemian pipeband (over 10,000 pipers and drummers I believe) is possibly the largest gathering of kilties.
