Canadian Kilt is referring to the hole that is sometimes found in the left lapel of jackets. It is where a bouttoniere is placed. This is a small bouquet or single flower that men wear for formal or dress occasions. The bouttoniere is placed through the hole and pinned on the back of the lapel.
