    Despite the general prohibition on discussions centering around modern firearms, I would suggest that you google "holster makers" and have them make you what is referred to as an inside the waistband (IWB) holster for your insulin pump. This will ride comfortably between you and the kilt, be totally discrete, and instantly accessible (just like my cordless remote hole puncher). The cost will also be a 100% tax write-off.

    Yep kinsman, good office equipment can be a real life saver!

    For the short term, perhaps running the hose out the top of the sporran and along the underside of the strap. A couple of leather keepers to keep it located and couple of dummy ones on the offside for visual balance might do the trick. It could transition up at the high point of the strap nearest your hip. You can also get narrow two sided velcro straps if you are in a hurry and don't want to mess with leather keepers.

    If the pump has a clip on it, wear it inside the waistband with the clip on the waistband. You can also have two buttons sewn on the inside of the kilt about 2 to 3 inches apart in a place that works best for you. Then use an equivalent length strip of double thickness leather with a button hole in each end. Button the leather to the kilt and clip the pump to the leather strip. This should allow you to hide both the pump and the hose. The strip can be used with multiple kilts and carried with your pump accessories.
