I've only worn mine while piping. I'd never wear it any other time. It'd feel too costume-y.

In Victorian times it was still the custom to hang on as many weapons and accoutrements as could be borne, apparently a holdover from when gentlemen went constantly armed in turbulent earlier times. I still remember the lines in the old 16th c. folk song "froggie went a courtin', he did ride, sword and pistol by his side..." Best not try reviving that style today.
In those days, a sword was the mark of a gentleman, and one could not be admitted to the formal presence of the King of France unless he was wearing one. Now heads of state try to discourage the carrying of weapons in their presence, and everywhere else for that matter.
Now this is my own opinion: a dress dirk looks good with a military-style doublet, as with pipers, but out of place with a civilian Prince Charlie or casual dress. The plain dirks would look all right in a Jacobite outfit, sort of Rob Roy-ish, but not with modern casual.

I would not wear a dirk these days. As has been mentioned it would feel too much like a costume. I do however where a Sgian Dubh, instead of carrying a pocket knife. It is very low key if worn properly and is quite utilitarian.

I wear it so that the handle protrudes just enough to grasp a hold of when I want to use the knife.

I've worn my dirk with my Prince Charlie for formal events. I use it to pop open champagne bottles. It's a real crowd pleaser.

Other than when piping in uniform, I'll occasionally wear one if I may need to dispatch a haggis or wedding cake; again, it can be a real crowd pleaser.
